Patient Rights
- Be informed of your rights and all regulations regarding your responsibilities as a patient, in writing, at the time of admission.
- Exercise your rights — or have your family or guardian exercise them on your behalf if you are judged incompetent.
- Participate in the planning of your home health services, and receive information on the scope of services offered.
- Be notified about financial benefits when referred to another organization.
- Be informed of your rights under state law to formulate advance directives.
- Choose a health care provider without discrimination and receive care according to your physician's orders.
- Receive advance notice of the type of care, the disciplines involved, and the frequency of visits.
- Be informed, orally and in writing, of any changes in payment sources and charges when they occur.
- Receive information about your medical condition and participate in care planning.
- Make informed decisions to accept or refuse treatment within the limits of the law.
- Learn treatment methods so you can self-manage your care when possible.
- Identify staff members through proper identification.
- Have your property and person treated with respect, consideration, and full recognition of your dignity.
- Voice grievances without fear of retaliation.

- Have your medical records kept confidential, with control over their release.
- Receive OASIS privacy rights information.
- Access and request amendments to your personal health information.
- Receive advance notice of service termination, with the right to appeal.
Patient Responsibilities
- Confirm your understanding of the care plan, verbally or in writing.
- Remain under a physician's care while receiving services.
- Meet physician appointment requirements for Medicare "face-to-face" compliance.
- Supply a complete and accurate health history.
- Report changes in your health status.
- Raise any concerns about the quality of your care.
- Ask questions to clarify your treatment.
- Report any medication-related issues.
- Provide insurance and financial information.
- Sign required consents and insurance releases.
- Authorize the agency to file payment appeals.
- Notify staff of physician-ordered changes to treatment.
- Participate actively in care planning, including pain management.
- Be available for scheduled home visits, and notify the agency when you are unavailable.
- Treat Miracle Home Health of Wisconsin staff with respect and dignity, without discrimination.
- Accept the consequences of refusing treatment.
- Maintain a safe home environment.
- Cooperate with your physicians and caregivers.
- Request clarification if you cannot understand written instructions.
- Provide a designated primary caregiver when staff are not present.
